Preoperative Chemothermoradiotherapy for Carcinoma of the Colon and Rectum

Abstract
Over the last 20 years there has been tremendous growth in our knowledge of the
basic biology of cancer. There have also been striking advances in the use of
technology. My specialty, radiation oncology, for example, has been virtually
revolutionized by the introduction and now clinical application of three dimensional
treatment planning which enables the radiation oncologist utilizing Imaging information
from CT and MR studies to precisely delineate the tumor and normal tissues and
deliver the appropriate radiation dosage to the former while minimizing dose to the
latter. These technological developments would not have been possible without the
advances in computer graphics and power that haute occurred over the last decade.
Nonetheless, our primary interest as clinicians remains what we can do for our
patients that will have the greatest impact on survival. Over the last two decades it
seems clear that the scientific advance that has had the greatest benefit for survival is
the intelligent use of combined modality therapy (CMT) for a wide variety of solid
tumors, particularly, however, of the gastrointestinal tract. Hopefully, knowledge that has
been gained from the treatment of GI tract malignancies may be brought to bear on the
treatment of other malignancies as well.
